Thank you for the work that you do each and every day for BPS students. As stated within our grant approval guidelines, we ask that you fill out this simple form to give the foundation a brief overview as to how your funding was utilized.

Grant Verification Form

This form is for grant applicant approvals to fill out once funds have been utilized. Deadline for spring (April-July) grant awardees will be the following December 31st. Deadline for fall (November-January) grant awardees is the following June 30th. If for some reason your verification form cannot be submitted within this timeframe please reach out the BPSF to review.
  • Accepted file types: jpg, png, pdf.
    Please provide a photo of your students utilizing the equipment or materials purchased through this grant. These photos will be utilized within our annual impact report and/or website to showcase how these funds are being utilized within our district. (If the photo cannot include your students for privacy reasons, please submit the photo of equipment and/or material with the staff that will be utilizing it within their classroom.)
